Welcome to Collinwood Elderly Care and Valley Park CBRF, providing care and services for senior citizens and/or disabled adults in the Brodhead, Orfordville and Albany, WI area.
COLLINWOOD & VALLEY PARK are State Licensed community
based residential facilities (CBRF) providing care and services for
senior citizens and/or disabled adults. Our homes, located in
Brodhead and Orfordville, and NOW OPEN in Albany, WI are
single-story, handicapped accessible structures. Private and
semi-private rooms accommodate ambulatory, semi-ambulatory
and non-ambulatory residents. Large dining room, family room
and visiting rooms provide a home-like atmosphere. A beauty
salon is also available to our residents.
Modern security equipment, smoke and fire alarms, resident call lights, sprinkler system and lighted hallways are in place to ensure residents safety and security.
At COLLINWOOD & VALLEY PARK, our residents are treated with the dignity and respect our senior citizens deserve. We are always open to suggestions from family and friends, relative to our residents' care.
Our services are specially designed to meet the needs of the adult with a dementia, such as Alzheimer's Disease; a physical disability, such as results from a stroke or Parkinson's Disease; or who because of chronic illness spent long periods of time in institutions and are now ready for a less restrictive setting.
We work closely with Hospice Care to meet the needs of terminally ill patients.
COLLINWOOD & VALLEY PARK have a basic rate for private and semi-private rooms. There is no "level of care" fee. A pre-admission assessment will be done by the Manager to determine suitability for admission to Collinwood. There is no charge made to the resident for this assessment.
COLLINWOOD & VALLEY PARK have contracts with Green and Rock County to accept county funding.
