COLLINWOOD is a State Licensed community based residential facility (CBRF) providing care and services for senior citizens and/or disabled adults. Our home, located in Brodhead, WI, is a single-story, handicapped accessible structure. Private and semi-private rooms accommodate ambulatory, semi-ambulatory and non-ambulatory residents. Large dining room, family room and visiting rooms provide a home-like atmosphere. A beauty salon is also available to our residents.
Modern security equipment, smoke and fire alarms, resident call lights, sprinkler system and lighted hallways are in place to ensure residents safety and security.

At COLLINWOOD, our residents are treated with the dignity and respect our senior citizens deserve. We are always open to suggestions from family and friends, relative to our residents' care.
Our services are specially designed to meet the needs of the adult with a dementia, such as Alzheimer's Disease; a physical disability, such as results from a stroke or Parkinson's Disease; or who because of chronic illness spent long periods of time in institutions and are now ready for a less restrictive setting.
We work closely with Hospice Care to meet the needs of terminally ill patients.
COLLINWOOD has a base room rate and each resident will be individually assessed to see if any additional services are needed to effectively care for them. Rates could vary some with increased needs. A pre-admission assessment will be done by the Manager to determine suitability for admission to Collinwood. There is no charge made to the resident for this assessment.
COLLINWOOD has contracts with all area Managed Care Organizations (MCO) which allow state funding to help those who need financial assistance. MCO's such as Inclusa, Care WI, and My Choice Family care to name a few, which cover the entire state of Wisconsin.
